The first-of-its-kind weight loss pill containing the GLP-1 agonist orforglipron

Skip to:

Orforglipron (orforglipron) is a medication, which is approved in the US as diet pill.

It works by activating the GLP-1 receptor, similar to the known medications containing semaglutide and tirzepatide.

But unlike them, he has a unique, non-peptide stricture, which allows for much better bioavailability, so that it can be taken at any time of the day, regardless of food or water intake.

In this article, we will reveal what the science says about Orforglipron:

  • How effective is Orforglipron?
  • Is Orforglipron safe?
  • How is Orforglipron dosed?

It is important to note that Orforglipron is available only by prescription and is only suitable for people with significant obesity and related diseases.

It is currently not available in Europe and a regulatory decision from the EMA (European Medicines Agency) is awaited for the European market.

What is the Orforglipron Weight Loss Pill?

Orforglipron is a synthetic small molecule originally developed by the Japanese pharmaceutical company Chugai Pharmaceutical Co., Ltd. (under the code OWL833), and subsequently licensed by the American Eli Lilly & Co. in 2018. [1],[2].

Unlike tirzepatide and semaglutide, which are peptides and are given via subcutaneous injections, orforglipron is a non-peptide small molecule that is taken in the form of a daily tablet.

The active ingredient orforglipron is built around a rigid, polycyclic heteroaromatic structure, including multiple aromatic and nitrogen-containing rings.

This structure allows it to be completely resistant to stomach acid and enzymes such as dipeptidyl peptidase 4 (DPP-4), which provides it with the following characteristics:

  • high oral bioavailability of about 77–79% [3],[4].
  • its half-life is approximately 29–49 hours [4].[5].
  • maintains effective blood concentration within 24 hours and allows once-a-day administration.

For comparison, oral semaglutide (Ribelus) is taken on an empty stomach with a minimal amount of water.

Orforglipron can be taken at any time of day, without restrictions regarding food or water – a convenience that significantly distinguishes it from oral semaglutide.

Mechanism of Orforglipron

Orforglipron activates the GLP-1 receptor, but in a fundamentally different way than peptide GLP-1 agonists.

While endogenous GLP-1 and peptide analogs such as semaglutide bind to the orthosteric (primary) binding site of the receptor, orforglipron acts as an allosteric agonist and binds to a transmembrane pocket. [1],[4].

This unique mode of binding stabilizes an active conformation of the receptor and induces preferential activation of Gs-protein signaling (and consequent accumulation of cAMP), while demonstrating minimal engagement of the β-arrestin pathway.

Simply put, orforglipron activates the GLP-1 receptor in a way that maximizes metabolic efficiency without inducing receptor internalization (habituation) [1],[6].

Activation of the GLP-1 receptor by orforglipron leads to:

  • Stimulation of glucose-dependent insulin secretion from pancreatic β-cells, supporting blood sugar control via a glucose-dependent pathway, without the risk of hypoglycemia.
  • Suppression of glucagon secretion from α-cells, reducing glucose production by the liver.
  • Delaying gastric emptying, which reduces postprandial blood sugar spikes.
  • Suppression of the centers in the brain that regulate appetite and reduce the desire for food.

Preclinical pharmacological studies demonstrate that the efficacy of orally administered orforglipron is comparable to that of injectable GLP-1 agonists such as exenatide and semaglutide in models of diabetes and obesity. [1],[6].

Another important advantage of orforglipron as a non-peptide molecule is its lack of immunogenicity – unlike peptide GLP-1 agonists, which can induce the formation of anti-drug antibodies, orforglipron does not provoke an immune response. [4].

What is Orforglipron Approved for?

Given that Eli Lilly is an American company, the first studies of the drug were conducted in the United States. After a series of clinical trials, including multiple phase 2 and phase 3 trials, the US Food and Drug Administration (FDA) approved the drug for obesity.

Specifically, Eli Lilly is investigating several applications for orforglipron in the following chronological order:

  • 2018 – Eli Lilly licenses orforglipron from Chugai Pharmaceutical. Preclinical data on the molecule are jointly published [1].
  • 2023 – Phase 2 results published in The New England Journal of Medicine (for obesity) and The Lancet (for type 2 diabetes), demonstrating significant weight loss to −14.7% and reduction in HbA1c to −2.1% [7],[8].
  • 2023 – Phase 3 of the ATTAIN (for obesity) and ACHIEVE (for type 2 diabetes) programs launch.
  • 2025 ACHIEVE-1 results published, positive results announced from ACHIEVE-3, ATTAIN-1, ATTAIN-2 and ATTAIN-MAINTAIN [9],[10],[11],[12].
  • April 2026 – Through the National Priority Program, the FDA approves orforglipron for the treatment of obesity in adults with a BMI ≥ 30 kg/m² or ≥ 27 kg/m² with at least one weight-related comorbidity [2],[13].

Currently, Orforglipron is FDA-approved only for weight management.

Approval for type 2 diabetes is expected upon completion of the ACHIEVE program. Regulatory decisions from the EAL and other regulatory authorities around the world are also awaited.

In addition to obesity and diabetes, orforglipron is also being studied for obstructive sleep apnea, hypertension, knee osteoarthritis, stress urinary incontinence, as well as cardiovascular and kidney problems. [2].

How Effective Are Orforglipron Weight Loss Pills?

The effectiveness of Orforglipron for weight loss has been studied in detail in the ATTAIN program – a series of phase 3 studies involving over 4,500 overweight or obese participants.

A key benchmark is the percentage of initial weight that patients lose during therapy. Below we will look at the key studies:

ATTAIN-1

ATTAIN-1 is the pivotal study that led to the FDA approval of Orforglipron. It enrolled 3,127 obese or overweight (non-diabetic) participants and lasted 72 weeks. [14].

The results including all participants are:

  • 6 mg/day – average weight loss −7.5% (−7.8 kg)
  • 12 mg/day – average weight loss −8.4% (−8.6 kg)
  • 36 mg/day – average weight loss −11.2% (−11.3 kg)
  • Placebo – −2.1% (−2.4 kg)

In participants who remained on therapy for the entire study, weight loss at 36 mg/day was −12.4% (−12.4 kg). Impressively, 59.6% of the 36 mg patients lost at least 10% of their weight, and 39.6% lost at least 15%.

In addition to weight loss, Orforglipron also significantly improved markers of cardiovascular risk, including waist circumference, non-HDL cholesterol, triglycerides, and systolic blood pressure at all doses.

The highest dose reduced markers of inflammation, such as high-sensitivity C-reactive protein (hsCRP), by 47.71%. [14].

ATTAIN-2

ATTAIN-2 evaluated the effectiveness of Orforglipron in 1613 participants with overweight/obesity and type 2 diabetes for 72 weeks [10].

The results are as follows:

  • 6 mg/day – average weight loss −5.1%
  • 12 mg/day – average weight loss −7.0%
  • 36 mg/day – average weight loss −9.6%
  • Placebo – −2.5%

At 36 mg, 45.61% of participants lost at least 101% of their weight (compared to 9.01% of placebo participants). All doses also significantly improved glycemic control (HbA1c), blood pressure, and lipid profile.

ATTAIN-MAINTAIN

The latest study from the program evaluates whether switching from injectable GLP-1 to oral Orforglipron can maintain achieved weight loss [12].

376 participants who had previously completed the SURMOUNT-5 trial of injectable tirzepatide and semaglutide were randomized to orforgliprone or placebo for 52 weeks.

The results showed that participants who switched from semaglutide to orforglipron maintained almost all of their weight loss, regaining an average of only 0.9 kg.

Those who switched from Zepbound to orforglipron regained an average of 5.0 kg. In comparison, the placebo groups regained an average of 9.1–9.4 kg over 24 weeks.

This study is of great clinical importance as it demonstrates the possibility of long-term maintenance therapy with a more convenient oral form after achieving target weight with injections.

How Does Orforglipron Compare to Other Diet Pills and GLP-1 Medications?

Although there is no direct comparative study between Orforglipron and injectable semaglutide and tirzepatide for weight loss, data from various clinical programs allow the following indicative comparison:

  • Tirzepatide 15 mg/week – average weight loss of about −20.9% over 72 weeks (SURMOUNT-1) [15]
  • Semaglutide 2.4 mg/week – average weight loss of about −13.7% over 72 weeks (SURMOUNT-5) [16]
  • Orforglipron 36 mg/day – average weight loss of about −11.2% over 72 weeks (ATTAIN-1) [14].
  • Oral semaglutide 25 mg/day – average weight loss of about −13.6% over 64 weeks (OASIS-4)

As can be seen, Orforglipron offers more modest weight loss compared to injectable forms.

However, the convenience of a daily tablet without restrictions regarding food and water, as well as the lack of the need for injections, may be an important advantage for some patients.

How Effective is Orforglipron for Diabetes?

Orforglipron also demonstrated significant efficacy in people with type 2 diabetes. These data were obtained from the ACHIEVE clinical program, which included over 6,000 participants in five phase 3 studies.

ACHIEVE-1

ACHIEVE-1 is a phase 3 study published in NEJM, involving 559 participants with early type 2 diabetes (controlled with diet and exercise alone) for 40 weeks. [9].

Results:

  • HbA1c reduction: −1.24% (3 mg), −1.47% (12 mg) and −1.48% (36 mg) compared to −0.41% with placebo.
  • The mean HbA1c at week 40 was 6.5–6.7% with orforglipron.
  • 68–731% of patients achieved HbA1c < 7.01% (vs. 331% on placebo).
  • Weight loss: −4.5% (3 mg), −5.8% (12 mg) and −7.6% (36 mg) vs. −1.7% for placebo.
  • No episodes of serious hypoglycemia were recorded.

ACHIEVE-3 – Comparison between diet pills Semaglutide and Orforglipron

The first head-to-head comparative study between two oral GLP-1 agonists – the diet pills containing semaglutide and orforgliprone, published in The Lancet in 2026, included 1698 participants with type 2 diabetes treated with metformin. [11].

Results after 52 weeks:

  • HbA1c reduction: −1.71% (orforglipron 12 mg), −1.91% (orforglipron 36 mg) versus −1.23% (oral semaglutide 7 mg) and −1.47% (oral semaglutide 14 mg).
  • Weight loss: −6.7% / −14.6 lbs (orforglipron 12 mg), −9.2% / −19.7 lbs (orforglipron 36 mg) vs. −3.7% / −7.9 lbs (semaglutide 7 mg) and −5.3% / −11.0 lbs (semaglutide 14 mg).

At the highest dose, orforglipron demonstrated 73.61% greater relative weight loss compared to oral semaglutide 14 mg (Ribelus).

Additionally, all doses of orforglipron also improved cardiovascular risk factors, including systolic blood pressure, triglycerides, and cholesterol.

What Other Benefits Does Orforglipron Have?

In addition to weight loss, orforglipron demonstrated significant positive effects on cardiovascular risk markers.

An exploratory analysis of data from the phase 2 program, published in Cardiovascular Diabetology, evaluates the effect of orforglipron on multiple cardiovascular biomarkers in participants with type 2 diabetes and/or obesity. [17].

Statistically significant improvements were found in:

  • Systolic blood pressure: decrease by −7 to −9 mmHg
  • LDL cholesterol: reduction to −17%
  • Triglycerides: decrease to −20%
  • Apolipoprotein B and apolipoprotein C3: significant reduction
  • High-sensitivity C-reactive protein (hsCRP): decrease by 26–42%

The reductions in hsCRP were of a magnitude historically associated with cardiovascular benefits in studies of injectable GLP-1 agonists. In the ATTAIN-1 study, at 36 mg hsCRP was reduced by 47.71% (3T). [14].

One caveat is that orforglipron increases heart rate by about 3–7 beats/min, which is typical of the GLP-1 class and should be monitored. [11],[17].

A 2026 meta-analysis summarized data from four large trials and confirmed that orforglipron provides consistent and significant improvements across the spectrum of risk factors for atherosclerotic cardiovascular disease. [18].

How is Orforglipron Dosed for Weight Loss?

Based on data from the ATTAIN and ACHIEVE programs, as well as FDA approval, there are currently clear dosing recommendations for Orforglipron. [2],[5].

Orforglipron is effective when taken orally (as a weight loss pill), once a day, at any time of the day. There are no restrictions on food or water intake – it can be taken with or without food.

But as already mentioned, in clinical studies, doses have been tested as weight loss capsules in amounts up to 36 mg.

According to Eli Lilly & Co., the dosages of weight loss capsules correspond to different doses when the form is tablet.

Orforglipron is available as weight loss tablets in the following doses: 0.8 mg, 2.5 mg, 5.5 mg, 9 mg, 14.5 mg and 17.2 mg. These correspond to the following capsule doses of 1 mg, 3 mg, 6 mg, 12 mg, 24 mg and 36 mg.

The FDA approves the drug in the form of tablets with equivalent doses of up to 17.2 mg, which provide the same concentrations of the drug in the body. [19].

The dosing regimen provides for a gradual increase in dose every 4 weeks to minimize gastrointestinal side effects:

Specifically, the equivalence is as follows:

  • 0.8 mg/day for 4 weeks
  • 2.5 mg/day (5–8 weeks)
  • 5.5 mg/day (9–12 weeks)
  • 9 mg/day (13–16 weeks)
  • 14.5 mg/day (17–20 weeks)
  • 17.2 mg/day (after 21st week)

It is not necessary to increase the dose every 4 weeks if the current dose is effective. However, it is important not to increase the dose before at least 4 weeks of the current step have been taken.

Risks and Side Effects of Orforglipron

Orforglipron has been studied in multiple phase 2 and phase 3 clinical trials that also evaluated potential side effects.

A 2024 meta-analysis encompassing data from randomized controlled trials summarizes the drug's safety profile [20].

The most common side effects are gastrointestinal and are usually mild to moderate in severity, occurring primarily during the dose escalation period. According to data from ATTAIN-1 at a dose of 36 mg [14]:

  • Nausea – 33.7% (vs. 10.4% on placebo)
  • Constipation – 25.4% (vs. 9.3% on placebo)
  • Diarrhea – 23.1% (vs. 9.6% on placebo)
  • Vomiting – 24.0% (vs. 3.5% on placebo)
  • Indigestion – 14.1% (vs. 5.0% on placebo)

Discontinuation of therapy due to side effects has been observed in:

  • 5.1% (6 mg), 7.7% (12 mg) and 10.3% (36 mg) from participants on Orforglipron
  • 2.6% in placebo

Is Orforglipron safe?

The most common side effects associated with the use of Orforglipron affect the gastrointestinal tract and digestion, which is typical of drugs that activate GLP-1 receptors.

The main reason for these effects is the activation of receptors in the brain associated with nausea, as well as a temporary delay in gastric emptying, which is observed at the beginning of therapy and when the dose is increased.

For this reason, nausea may be observed primarily after feeding. Since this delay is transient, the side effects are usually temporary.

It is important to note that no hepatotoxic signal was detected in clinical studies – which is of particular importance for a small molecule, as they are typically metabolized in the liver. [2],[19].

Serious side effects may include excessive weight loss (and, very rarely, pancreatitis), allergic reactions, and vision changes in people with diabetes.

As with other GLP-1 agonists, an increased risk of thyroid tumors (C-cell tumors) has been observed in preclinical studies in rodents.

For this reason, Orforglipron is contraindicated in people with a personal or family history of medullary thyroid carcinoma (MTC) or multiple endocrine neoplasia type 2 (MEN 2) syndrome. [2].

A serious and often underestimated risk is the loss of muscle mass, which is a direct result of suppressed appetite.

This problem can be prevented by combining drug therapy with adequate diet rich in protein, and most of all weight training.

How to Store Orforglipron?

Orforglipron is an oral medication for daily use that is available only by prescription in the form of film-coated tablets.

One of the significant conveniences of Orforglipron is that it can be stored at room temperature – it does not require refrigeration. [2].

This is a significant advantage compared to injectable GLP-1 agonists such as tirzepatide and semaglutide, which require refrigerated storage and careful handling during transportation.

Store the tablets in the original packaging, in a dry place protected from light, at room temperature.

What is the Difference Between Orforgliprone and Tirzepatide?

Orforgliprone and tirzepatide are manufactured by the same company (Eli Lilly), but they differ significantly:

Characteristics

Orforglipron

Tirzepatide

Molecule type

Non-peptidic small molecule

Peptide (39 amino acids)

Method of administration

Tablet, once daily

Subcutaneous injection, once weekly

Targets

GLP-1 receptor only

GLP-1 + GIP receptors (dual agonist)

Average weight loss (without diabetes)

−11.2% for 72 weeks (17.2 mg/day)

−20.9% for 72 weeks (15 mg/week)

Admission restrictions

No food/water restrictions

No food/water restrictions

Storage

Room temperature

Refrigerator (2–8°C)

Immunogenicity

None (not a peptide)

Very rare but possible allergic reactions (peptide)

Tirzepatide is more effective for weight loss due to dual GLP-1/GIP activation, but orforgliprone offers the unique convenience of oral administration without injections.

Compared to other diet pills, the medication has no food or water restrictions and does not require refrigeration.

Opinions about Orforglipron from a doctor:

„"Orforglipron is inferior in terms of weight loss effectiveness to the already approved medications containing semaglutide and tirzepatide.".

But unlike them, the medication is available in oral form and can be taken at any time of the day, regardless of food and water intake.

This provides convenience that is not offered by any other diet pill.

The side effects of the drug are absolutely comparable to those of semaglutide and tirzepatide, and can be almost completely avoided when the therapy is carried out under the supervision of an experienced physician.“
